Patient Portal Sign-up Instructions

Complete the following steps to connect with us online:

1. Visit or call our clinic and provide your email address to receive a portal invitation.
2. Follow the link to set up your account.
3. Once an account is set up you can access it through the Patient Portal link on the top right of our website.
4. Congratulations! You have successfully created a secure account linked to your medical record. You can now access the great features online with EMA.
